Comments:
Licensing inspector arrived at 1:06 pm and departed at 3:10 pm to conduct a licensing inspection and a Subsidy Health and Safety inspection. At the time of the tour there was thirty-six children in care with staff . Additional staff was present to assist in the classrooms. Children were observed napping, diaper changing, waking up, and watching movies. The sample size consisted of 5 children records and 5 staff records.

Standard #: 22VAC40-185-350-E-5
Description: Based on record review and staff interviews, the licensee did not ensure child ratio was maintained for the children, from age of eligibility to attend public school through eight years, one staff member for every 18 children. Evidence: During the inspection conducted on August 8, 2018, in the school age classroom there were 21 children from ages 5- 11 years of age with one staff . Confirmed with staff .

Plan of Correction: During transition after field trip and activity. The director stepped out and return a short time afterwards . There were plenty of staff in the building.

Standard #: 22VAC40-185-500-B
Description: Based on observation and interviews, the licensee did not ensure the diapering surface is cleaned with soap and at least room temperature water and sanitized after each use. "Sanitized" means treated in such a way to remove bacteria and viruses from inanimate surfaces through using a disinfectant solution (i.e., bleach solution or commercial chemical disinfectant) or physical agent (e.g., heat). The surface of item is sprayed or dipped into the disinfectant solution and allowed to air dry after use of the disinfectant solution. Evidence : During the inspection conducted on August 8, 2018, in the infant classroom, staff #1 completed changing a child and staff #2 brought another child to the diapering surface and changed the child. The diapering surface was not cleaned. After the second child was changed staff #2 sprayed and wiped surface immediately down.

Plan of Correction: Review with staff member proper procedures to clean diapering surface.
