22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 9:00 am-12:15pm. At the time of entrance, 22 children were in care with 7 staff members present. Children were observed during circle time, playing with legos, art activities and outdoor play time. Positive interactions were observed between children and staff. The school age program was heading on a field trip to the Potomac Nationals. A selection of staff and children records, emergency kit, and other documents were reviewed. Standard #: 22VAC40-185-140-A Description: Based on record review, the center failed to obtain a physical examination for each child within one month after attendance: Evidence: 1. Child #4 (start date 1/02/2018) was missing the child's physical examination and has attended more than one month. Plan of Correction: I will ask the parent for a copy of the child's physical.
Standard #: 22VAC40-185-160-C Description: Based on record review, the center failed to obtain results of follow up tuberculosis screening (TB) at least every two years for all staff members. Evidence: 1. Staff #1 had a TB screening dated 7/25/2016 and expired 7/25/2018. 2. Staff #3 had a TB screening dated 7/25/2016 and expired 7/25/2018. 3. Staff #4 had a TB screening dated 7/25/2016 and expired 7/25/2018 Plan of Correction: I will get a copy of their updated screenings.
Standard #: 22VAC40-185-60-A Description: Based on record review, three of five children's records did not have complete information. Evidence: 1. Child #1 and child #5 were missing previous child day care and schools attended by the child. 2. Child #4 (start date 1/02/2018) was missing the child's physical examination. Plan of Correction: I will ask the parents for the information.
Standard #: 22VAC40-185-270-A Description: Based on observation, areas and equipment of the center inside were not maintained in a safe and operable conditioni. 1. In the blue classroom, a cabinet door had a hinge that has become unattached. Plan of Correction: A work order has already been placed and will be repaired.
Standard #: 22VAC40-185-290-3 Description: Based on observation, electrical outlets were not all covered with protective covers. Evidence: 1. In the green room, two unused outlets were not covered with protective covers to the left of the door. 2. In the white room, two unused outlets were not covered with protective covers, 2 unused outlets on a surge protector were not covered and one outlet was not covered to the right of the door. Plan of Correction: This will be corrected today.
Standard #: 22VAC40-185-560-G Description: Based on observation, the center did not follow all requirements when food is brought from home. Evidence: 1. In the blue classroom, Child #1 and Child #5 had food from home in lunch bags in the classroom that were not dated. Plan of Correction: We will make sure the label is on today and in the future.
