Technical Assistance:
Reviewed the renewal application with center owner.

Today an unannounced Renewal Inspection was conducted between the hours of 11:00am and 3:00pm. There were 7 classrooms observed with a total of 57 children with 11 staff within the supervision guidelines. The children were observed eating lunch, playing with play doh, playing in small groups and getting ready for nap. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Standard #: 22VAC40-185-130-B
Description: Based on record review, the center did not obtain additional immunizations every six months for children under the age of two. Evidence: Child #1 is approximately 1.5 years old and their immunizations from when they were 10 weeks old. Child #2 is approximately 1.5 years old and their immunizations are from when they were 6 months old.

Plan of Correction: Child #1 has a doctor's appointment tomorrow and I'll remind parents to bring updated records. I'll remind the parents of Child #2 that we need updated records.

Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records did not contain all required information. Evidence: Child #1 file did not contain an address for their first emergency contact. Child #2 file did not contain an address for their first emergency contact and did not contain a second emergency contact. Child #3 file did not contain an address or phone number for their first emergency contact and did not contain an address for their second emergency contact.

Plan of Correction: I'll get those updated but I've sent new enrollment packets to all the parents and we'll ensure that they are filled out completely.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas of the centers were not maintained in a safe condition. Evidence: At the time of the inspection, there was a long hose piled up on the toddler playground that stretched over to the back playground for the children to do water play. The section of the hose that is on the toddler playground has created a tripping hazard.

Plan of Correction: I'll purchase a hose storage system so that the teachers can roll the hose up when it's not in use.

Standard #: 22VAC40-185-280-D
Description: Based on observation, cleaning materials were located above food. Evidence: There are various cleaning materials stored on a shelf above food in the closet of the infant classroom.

Plan of Correction: I will talk to the teachers again and I'll double check that it has been done.

Standard #: 22VAC40-185-570-E
Description: Based on observation, bottles were not labeled with today's date. Evidence: 3 out of 10 infant bottles were not labeled with a date.

Plan of Correction: The teacher's are usually good about double checking the bottles in case parents forget. I'll remind the parents and the teacher's that it needs to be labeled.
