Comments:
An unannounced monitoring inspection was conducted from 9:00 am-1:15pm. At the time of entrance, 33 children were in care with 8 staff members present. Children were observed reading books, having diaper changes, playing outside and dancing to music. The infants were playing with toys, swinging, bouncing, crawling, walking, being held and bottle fed. A selection of staff and children records, and other documents were reviewed. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, three of five children's records did not have complete information. Evidence: 1. Child A was missing the child's allergy information, previous child day cares attended by the child and documentation of child updates and confirmation of up to date information. 2. Child B was missing the child's allergy information and previous child day cares attended by the child. 3. Child C was missing one parent's name, address, phone number, place of employment and work number.

Plan of Correction: I will ask the parent's for the information and review the other children's file to make sure they are up to date and not out of compliance.

Standard #: 22VAC40-185-70-A
Description: Based on record review, staff records did not include all required information. Evidence: 1. Staff #1 was missing documentation of 2 or more references and began at the center on 8/10/2018. 2. Staff #4 was missing documentation of 1 reference and began at the center on 9/27/2017.

Plan of Correction: I will make sure I get the references.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In the giraffe classroom, an odor eliminator was on top of the sink where children wash their hands and was not kept locked. 2. In the ducks classroom, an odor eliminator and a bottle of febreze spray was on top of the sink where children wash their hands. 3. In the penguins classroom, a container of comet cleaner and 2 febreze sprays were under the children's sink in an unlocked cabinet. The cabinet had a lock that was dangling on one knob, but not in the locked position over two knobs. There was a bottle of bleach water and another febreze spray on a shelf above the sink.

Plan of Correction: I will make sure the cleaning products are locked at all times.

Standard #: 22VAC40-185-290-3
Description: Based on observation, all unused electrical outlets were not covered with protective covers. Evidence: 1. In the penguin classroom, one unused outlet was not covered on the right side wall under the green paint. 2. One unused outlet in the empty classroom outside of the penguin classroom was not covered. 3. One unused outlet in the infant room below the nursery rhyme wall was not covered.

Plan of Correction: I will cover the outlets that are not covered.

Standard #: 22VAC40-185-500-A
Description: Based on observation, staff did not follow all hand washing requirements. Evidence: 1. Staff #1 was observed in the Ducks classroom changing diapers. Staff #1 did not wash their hands with soap and running water before and after changing Child #1, Child #2 and Child #3's diapers. Child #1, Child #2 and Child #3 did not have their hands washed after diapering.

Plan of Correction: I will retrain the staff.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center did not meet all diaper procedures. Evidence: 1. During the inspection, the inspector observed Teacher #1 diapering child #1, child #2 and child #3 in the Ducks classroom and did not clean the diapering surface with soap and at least room temperature water and sanitized after each use.

Plan of Correction: I will retrain staff.

Standard #: 22VAC40-185-570-E
Description: Based on observation, infant formula bottles was not dated as required. Evidence: 1. Infant A had three bottles in a bag labeled with the infants name on it, but did not include a date on the bottles. 2. Infant B had four bottles in a bag labeled with the infants name on it, but did not include a date on the bottles. 3. Infant C had a bottle sitting in the bottle warmer labeled with the infants name on it, but did not include a date on the bottle.

Plan of Correction: I will make sure the staff are trained to label every bottle with full name and date.
