Today an unannounced Monitoring Inspection took place between the hours of 9:50am and 2:15pm. At the start of the inspection there were a total of 72 children with 9 staff; all ratios were incompliance. A total of 10 children?s files and 5 staff files were reviewed. The infants were observed playing on the floor and sitting in bouncy chairs. The older children were enjoying outdoor water play and free play inside. Standard #: 22VAC40-185-60-A-7 Description: Based on record review, children's records did not include actions to take regarding food allergies in an emergency situation. Evidence: Child #9, Child #10 and Child #11 physical exams stated that the children had allergies to food. All three children's names are listed on classroom "Food Allergy" lists. All three files did not contain an action to take in case of accidental exposure or ingestion of the foods. Plan of Correction: We'll speak to the parents to request doctors notes on the status of the children's allergies.
Standard #: 22VAC40-185-500-A Description: Based on observation, proper hand washing procedures were not used. Evidence: One child was observed having their diaper changed in the Toddler classroom. The teacher did not wash the child's hands after their diaper was changed. The teacher did not was their own hands prior to beginning to change the next child. Plan of Correction: We will retrain the teachers on proper hand washing.
Standard #: 22VAC40-185-500-B Description: Based on observation, diaper changing surfaces were not sanitized after each use. Evidence: Two children were observed having their diapers changed in the Toddler classroom. The changing table was sanitized in between each diaper change. Plan of Correction: We'll discuss proper sanitary with staff.
Standard #: 22VAC40-185-520-A Description: Based on observation, over-the-counter skin products were kept beyond their expiration date. Evidence: Child #6 sunscreen expired 5/2018. Child #7 bug repellent expired 11/2017. Child #8 sunscreen expired 12/2017. Plan of Correction: We'll throw away expired sunscreen and request replacements.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review, the center did not obtain central registry (CPS background check) results within the first 30 days of employment for each employee. Evidence: Staff #2 (date of hire: 6/13/2018) and Staff #3 (date of hire: 6/13/2018) files do not contain CPS background check results. Plan of Correction: We'll do them today.
