Comments:
The licensing inspector completed an unannounced monitoring inspection in response to a self-reported incident regarding the sight and sound supervision of children. The Department was notified of the incident on July 31, 2018. On August 13, 2018, the licensing inspector was on site from approximately 9:50am to 10:25am. Interviews were completed, observations were made, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports areas of noncompliance with the licensing standards related to the sight and sound supervision of children under 10 years of age. Violations:
Standard #: 22VAC40-185-340-F
Description: Based on interview and documentation review, the center did not ensure that children under 10 years of age are always within actual sight and sound supervision of staff. EVIDENCE: 1) In a written statement, Staff #1 stated that on 07/26/2018, she "...went into the bathroom to change another child's pull-up and I then heard screaming. I rushed out to see what was going on and I saw that [Child #1] had her arms wrapped tightly around [Child #2's] neck." 2) During interview on 08/13/2018, Staff #2 reported that Staff #1 was terminated from employment on 07/26/2018 as a result of not maintaining sight and sound supervision of children in her care. During nap time, Staff #1 went into the bathroom inside the classroom to change another child's pull-up leaving the remaining children out of sight and sound supervision because no other staff were present in the room. The children in the classroom were 2-years of age. Children under 10 years of age always shall be within actual sight and sound supervision of staff.

Plan of Correction: Per the Director: The staff member involved is no longer an employee at COA. To prevent future occurrences, all existing staff will be trained on sight and sound supervision at our next staff meeting on 8/28/18. All new staff will undergo sight and sound training at the time of their orientation. All staff, existing and future, will be instructed to call for managerial assistance should diapering or other daily activities compromise the employees' ability to maintain sight and sound.
