Comments:
An unannounced monitoring inspection was conducted on 08/17/2018 from 12:45pm - 3:45pm. At the time of entrance there were 28 children in care with five staff. The children ranged in age from three years - twelve years. The preschool class was observed during nap and afternoon free play. The school age class was observed during quite reading, playing a large motor game in the gym and during afternoon snack consisting of apples and milk. Two school age children were observed arriving from public school at approximately 3:00pm. One medication review was conducted. The sample size included four staff and eight children's records. The violation is listed on the violation notice and was reviewed with the program director at the conclusion of the inspection.

Standard #: 22VAC40-185-140-A
Description: Based on record review and interview, the center failed to obtain a physical exam by or under the direction of a physician prior to attendance or within one month of attendance. Evidence: 1. There was no documented physical exam on file for child 1 who was in care during the inspection. a. Child 1 has an enrollment date of 09/06/2016, 2. Administrative staff confirmed that a documented physical was not on file for child 1.

Plan of Correction: The parent will be asked to provide documentation of a current physical by next Tuesday. A physical was obtained for child 1 however, it has been misfiled.
