An unannounced complaint investigation was conducted regarding concerns of supervision and programming. Staff interviews were conducted, observation of the Nature Center classroom and program equipment was made. and review of email communication was made. The preponderance of evidence supports a valid finding regarding programming. The allegation of supervision was determined not valid. Thank you for your cooperation today. Description: Based on staff interviews, documentation and observation, materials were available to a child which was not age and stage appropriate. Evidence: On August 6, 2018, Child #1 placed a Perler bead up their nose, requiring medical attention. The manufacturer's recommended age for using Perler beads is age 6 years and up. Child #1 is 3 years old.

Plan of Correction: All staff will be trained on the recommended age ranges for all equipment, supplies and products maintained at the center. All classrooms will be reviewed to ensure that no equipment, supplies or products that are not appropriate for the specific ages of each classroom is found.
