Technical Assistance:

The licensing inspector completed an unannounced monitoring inspection in response to a self-reported incident regarding the care and protection of children. The Department was notified of the incident on July 30, 2018. On August 6, 2018, the licensing inspector was on site from approximately 1:03pm to 1:53pm. Interviews were completed, video footage and observations were made, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports areas of noncompliance with the licensing standards related to the care and protection of children. Standard #: 22VAC40-185-340-A
Description: Based on facility video review, interview, and documentation review on 08/06/2018, the center did not ensure that when staff were supervising children, they shall always ensure their care and protection. EVIDENCE: 1) The licensing inspector viewed facility video footage of the toddler classroom from 07/27/2018 and observed the following: a) At approximately 11:55am, Staff #1 was observed pulling a cot with her foot and dragging it to another area of the classroom while three children were standing on it. When Staff #1 pulled the cot and dragged it, Child #1 (18 months old) staggered backwards into the wall and hit her head. b) Staff #1 was observed looking directly at Child #1 as she fell to the ground, but continued setting down cots. Staff #1 did not check on Child #1 after she fell and hit her head. 2) The accident/injury report given to Child #1?s parents, dated 07/27/2018, states the following: ?[Child #1] was standing on a cot, when [Staff #1] pulled the cot out from under her. [Child #1] fell back against the wall and hit her head.?

Plan of Correction: Staff #1 was relieved of her duties for not complying with standards and company policy. Management will ensure all staff will be retrained on positive guidance practices in the classroom. Management will continue to emphasize the importance of contacting management immediately when feeling overwhelmed or frustrated.
