Comments:
The inspector conducted an unannounced mandated renewal inspection from 10:20 until 2:00 p.m. Upon arrival at the center, a class was observed picking vegetables (swiss chard) from the vegetable garden. There were 24 infants and children in care with staff supervising at all times. The administrative assistant was also present and kitchen staff. Infants were observed being fed, playing, and having a diaper change. Children were observed playing outside, preparing for lunch, eating lunch, and during rest time. Five children?s records and five staff records were reviewed. Violations from the last inspection had been corrected. Areas on non-compliance are identified on the violation notice. Standard #: 22VAC40-185-240-C
Description: Based on record review, staff did have the required annual training hours. Evidence: 1. The inspector reviewed five staff records. Two of five records (staff #3 and #4) did not have the required 16 hrs. training.

Plan of Correction: In the future, all staff will receive 16 hrs. of training per year.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to maintain the center in clean condition. Evidence: 1. The inspector observed a red metal piece of play equipment on the playground that was rusted and peeling paint. 2. The inspector observed a red foam climber that had foam bulging out and and black tape on it that was not adhering to the climber.

Description: Based on observation and interview, medication was not labeled with the child's name, name of the medication, the dosage amt., and the time to be given. Evidence: 1. The inspector observed a medication in a back pack that was not labeled with any of the required information.

Plan of Correction: The parent had brought in the medication that morning; the parent will be contacted to get the label and required information.

Standard #: 22VAC40-185-560-F-4
Description: Based on observation, children were being served a potential choking hazard. Evidence: 1. The inspector observed raw baby carrots being served to the preschoolers and to the two year old classroom.

Plan of Correction: The director stated they would not be served to children 3 and under in the future.
