Technical Assistance:
Technical assistance was provided in the following areas of the standards: 22VAC40-191 (Background checks); 22VAC40-185-(3)-210 (Program Leader qualifications); 22VAC40-185-(4)-330 (Play areas)

Comments:
An unannounced monitoring inspection was conducted on 8/24/18 from 10:00am - 12:30pm. At the time of entrance there were 19 children ages four moths old to seven years old in care with five staff. Children were observed participating in various activities in the classrooms playing outside, eating lunch and during rest period. Records were reviewed for five children and five staff during the inspection. There is currently no medication at the facility. Emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a review of five children's records, it was determined that the facility did not ensure that each child in attendance had a completed physical within one month of attendance. Evidence: 1. The record for child #3 (date of enrollment 11/2/16) did not contain a physical examination. 2. Staff #6 (Assistant Director) confirmed that the record for child #3 did not contain a physical examination.

Plan of Correction: The facility responded: Child #3 previously had a physical, but the parents of child #3 will be asked to provide a new copy of a physical examination. All newly enrolled children will have documentation in the their within one month of attendance.

Standard #: 22VAC40-185-160-A
Description: Based on a review of five staff records, it was determined that the facility did not ensure that each staff member shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing. Evidence: 1. The record for staff #1 (date of hire 4/2/18), working during the inspection, contained documentation of a negative tuberculosis screening that was completed on 5/20/18. 2. Staff #6 (Assistant Director) reviewed the record for staff #1, and confirmed there the tuberculosis screening was completed more than 21 days after staff #1 began employment.

Plan of Correction: The facility responded: Going forward all new staff will complete a TB screening within 21 days of employment.

Standard #: 22VAC40-185-380-A
Description: Based on observation and interview, it was determined that the facility did not ensure that there is a posted daily schedule that allows for flexibility as children's needs require. Evidence: 1. There was no posted daily schedule in the Pre-K classroom. 2. Staff #1 confirmed that there was no posted daily schedule in the Pre-K classroom.

Plan of Correction: The facility responded: There was a daily schedule posted in the Pre-K classroom, but it must have been taken down. A new daily schedule was posted in the Pre-K classroom during the inspection.
