63.2 Facilities and Programs..

Comments:
An unannounced monitoring inspection was completed on 08/30/2018. There were 12 children, ages 3 year-4 years, and two staff providing direct care and supervision present during today's inspection. The inspector reviewed 5 children's records during the on-site inspection. Full staff records will be reviewed at a later date as the staff records are kept off-site at the corporate office. There is no medication currently being administered at the facility. The inspector observed the children in the following activities: arriving to the program, carpet/circle/learning time, music and movement, during morning snack and playing in centers. Proper hand washing and table sanitization were also observed during the inspection. The inspector discussed the following with program staff: children's and staff records, emergency requirements, training for new staff positions, and the storage of cleaning materials. The inspector reviewed the facility emergency procedures and emergency supplies. The inspector arrived at 8:25 a.m. and departed at 11:00 a.m. Thank you for your time today. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review and discussion with staff, the facility failed to ensure that all children's records contain all components as required by the standards. Evidence: 1. The record for Child #1 did not contain the health information as required by 22VAC40-185-130 nor the documentation of viewing proof of the child's identity and age. 2. The record for Child #3 did not contain the following information: the work phone number of each parent; a place of employment was listed nor the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached; the addresses were incomplete for both people listed. 3. The record for Child #4 did not contain the following information: the name and phone number of the child's physician, names of persons authorized to pick up the child, health information as required by 22VAC40-185-140, written agreements between the parent and the center as required by 22VAC40-185-90, previous child day care, name of any additional programs or schools that the child is concurrently attending, documentation of viewing proof of the child's identity and age, and the first date of attendance.

Plan of Correction: The facility director called the Family Service Worker to request the information during the inspection.

Standard #: 22VAC40-185-70-A
Description: Based on record review and discussion with the staff, the facility failed to ensure that staff information, required by the standards, shall be kept at the center. Evidence: Staff #2 did not have the name, address and telephone number of a person to be notified in an emergency at the center; the standards require this information shall be kept at the center. Staff #2 was working during the inspection.

Plan of Correction: The facility director called the Family Service Worker to request the information during the inspection.

Standard #: 22VAC40-185-540-E
Description: Based on observation and discussion with the staff, the facility failed to maintain all nonmedical emergency supplies as required by the standards. Evidence: There was not a working battery operated radio available during the inspection.

Plan of Correction: Batteries were brought to the center during the inspection.
