Comments:
Today an unannounced monitoring inspection was conducted starting at 2:00pm and ended at 3:45pm. There were 59 children present today with 4 staff. Children were observed arriving from school, going over new program rules with the staff, washing hands, eating snack (cinnabars & 1% milk). One medication and authorization was reviewed. Medication Administration Training (MAT) and First Aid/CPR certifications were checked today. Staff records are reviewed annually in the administrative office. Violations:
Standard #: 22VAC40-185-240-D-1
Description: Based on observation and interview, the center has agreed to administer a prescribed medication to one child without a staff member on-site that has satisfactorily completed a training that qualifies them to administer the medication. Evidence: Child #1 has an emergency medication on-site to be administered in case of an accidental ingestion of an allergen. There are no staff members on-site that have been trained to administer this particular medication. Staff #1 thinks that she has completed the training but can't remember.

Plan of Correction: I'll check to see if I have completed the training.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place. Evidence: There are three bottles of sanitizer cleaner on an open shelf in the cafeteria where the program is taking place.

Plan of Correction: We will lock it up now.

Standard #: 22VAC40-185-350-E-5
Description: Based on observation and interview, there was not one staff member for every 18 children. Evidence: At approximately 2:45pm, there were 55 children present with three staff members. Staff #2 stated that their fourth staff person had called out of work and that another staff member from a separate office should be coming to help. Staff #5 arrived at approximately 2:57pm.

Plan of Correction: Today's shortage was due to a staff member calling out. We will have a discussion with all staff about providing more notice so that we can get a sub in quicker.
