Technical Assistance:
Consultation was given on additional MAT training if an Auvi-Q epi-pen is to be administered, ratios and background checks.

Comments:
An unannounced complaint investigation was conducted today on care of children. Based on the preponderance of evidence, no violations are issued. In addition a monitoring visit was conducted. Observed all rooms in ratio with direct care staff, playtime outside on the playground, transition from am breakfast (biscuit and scrambled egg and strawberries), infant care and free play. Reviewed mediction authorizations and storage, emergency preparation, trainings and 2 staff records. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-270-A
Complaint related: No
Description: Based on observation, areas and equipment throughout the center are not maintained in a clean, sfe or operable condition. Evidence: 1. Peeling paint was observed in the infant classroom by the gate between the two areas, in the hallway leading to the back of the building and in the Toddlers's classroom under the clock. 2. The tint film on the windows and front door is peeling off in the infant area windows, front door and toddler area windows. 3. A trike seat was cracked and posed a pinching hazard. 4. The smoke detector in the attic was "chirping" and needs a new battery.

Plan of Correction: The trike was removed from the playground during the inspection. A work order for the film and smoke detector has been placed and an additional one will be requested for the paint.

Description: Based on observation, an outlet was missing a protective cover. Evidence: The outlet in the hall by the green chair was uncovered.

Plan of Correction: A protective cover was placed in the outlet during the inspection.

Standard #: 22VAC40-185-340-D
Complaint related: No
Description: Based on center administration statement, a lead teacher supervises more than 2 aides/assistants. Evidence: The Toddler classroom has four teachers and only one meets lead teacher qualifications. The teacher is supervising three assistants/aides.

Plan of Correction: The center is in the hiring process and will hire an additional lead teacher for that classroom.
