22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted today from 9:00am-12:00pm. There were 44 children supervised by 8 direct care staff. The physical plant, programming, staff records, children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, and eating breakfast. There was an abundance of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Standard #: 22VAC40-185-60-A Description: Based on review of five children?s records, the center did not maintain complete children?s records. Child #4, start date not documented in file, last update 9/1/17. Plan of Correction: I will document her start date in her file.
Standard #: 22VAC40-185-270-A Description: Based on observation, not all areas of the center are maintained in a clean, safe and operable condition. Evidence: In the Intermediates, Pre K1, PreK2, and Beginners classrooms, chipped and peeling paint was observed accessible to children. In the Beginners classroom, the floor tile is lifting, which could cause a tripping hazard to children. Also in the Beginners classroom, the molding is pulling away from the wall, exposing the wall and broken blinds with hanging string were observed. On the back of the building, peeling and chipped paint was observed and is accessible to children. Plan of Correction: I have already contacted home office regarding the maintance issues. He has repaired some and will be back to complete the rest.
Standard #: 22VAC40-185-290-3 Description: Based on observation, the provider failed to cover all unused electrical outlets accessible to children under five years of age with child-resistant protective covers. Evidence: The power strip in the Pre K2 classroom and an outlet in the Intermediate classroom were uncovered and accessible to children. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-330-B Description: Based on observation of the playground, there is an insufficient amount of mulch. Evidence: The fall zones on both playgrounds are exposed and do not have sufficient mulch to protect a child in the event of a fall. Plan of Correction: We received new mulch in the summer, I will re-rake the mulch to protect the fall zones.
Standard #: 22VAC40-185-420-E-3 Description: Based on review of children's records, the center did not obtain annual updates for all records. Evidence: Children's records were not updated annually: Child #1, start date: 9/2/16, last update 1/3/17. Child #2, start date: 10/18/16, last update 10/5/16. Child #3, start date 8/28/15, last update 6/15/16 Plan of Correction: I will have the parents update their children's files.
Standard #: 63.2-1722-B Description: Based on staff record review, the center failed to obtain a background check for all employees. Evidence: Staff # 2, who lived outside the state of Virginia within the last five years, does not have an out of state background check on record. Plan of Correction: I will send out the out of state background check.
