Today an unannounced monitoring inspection was conducted starting at 2:55pm and ended at 4:05pm. There were 30 children present today with 2 staff. Children were observed eating an afternoon snack (goldfish and capri sun juice), and working on homework. Children that did not have homework played quietly (lincoln logs, coloring, table top games) in small groups. Four medications with authorizations were reviewed. Medication Administration Training (MAT) and First Aid/CPR certifications were checked today. Staff records are reviewed annually in the administrative office. Description: Based on a review of medications and authorizations, a parent's authorization for medication has expired. Evidence: Child #5 has emergency medication available on site in case of accidental ingestion of an allergen. The child's parents signed the medication authorization on 8/21/2018. A parent's medication authorization is good for 10 days.

Plan of Correction: I will have the parents get the rest of the form filled out.
