An unannounced monitoring inspection was conducted on September 6, 2018. There were six children present with two staff members. An additional volunteer was on site for support purposes. The posted menu indicated that for lunch, the children would be served chicken tenders, steamed broccoli, mixed fruit, bread and milk. Observations included the physical plant, nutrition, supervision, staff/child ratios, health and safety, and the children's records. The children were observed doing a preposition parachuting group activity, listening to a story and playing in centers. Six children's records were reviewed. There is currently no medication in the center. The outdoor playground equipment is not being utilized at this time.
Standard #: 22VAC40-185-60-A Description: Based on record review, in two of six children's records reviewed, the licensee did not ensure that the center would maintain and keep at the center a separate record for each child enrolled which shall contained all required information documented. Evidence: During the inspection conducted on September 6, 2018, the following information had not been documented in each child's record: Child #3-the place of employment for one parent Child #5-there was not an established record available to review; the child's record was faxed from another location Plan of Correction: Will contact the child's parent to get the name of the place of employment.
