22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 11:30am through 1:00pm. There were 6 children in care with three teaching staff. Children present ranged in age from 15months to four years old. Children and new staff records were reviewed. Children were observed in teacher and self-directed activities including playing with toys, transitions, restroom routines and hand washing. The first aid kit, required postings and outside play area were reviewed. There was one violations identified in the area of staff records.
Standard #: 22VAC40-185-70-A Description: Based on record review and interview staff record information has not been kept as required. Evidence: 1 - The record for staff 2 does not contain documentation of reference checks. a. Staff 2 has a documented date of hire as 5/25/18. 2 - Staff 1 stated that she did not know if the reference checks had been obtained or not. Plan of Correction: Center management will ensure that each staff records contains all required information including documentation of two reference checks for each staff prior to employment. Documentation for staff 2 will be obtained and present in the staff record by Friday of this week.
