Comments:
An unannounced monitoring inspection was conducted from 9:07 am-12:00pm. At the time of entrance, 12 children were in care with 4 staff members present. Children were observed washing hands and preparing for snack time, engaged in group activities at circle time and outdoor play time. There were no medication on site today. Staff with CPR/First Aid Training and MAT training present during the inspection. A selection of staff and children records and other documents were reviewed. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records, the center did not obtain a physical examination for each child within one month of attendance. Evidence: 1. Child B began at the center on 5/09/2018 and did not have a physical examination on file for the child.

Plan of Correction: I will ask the parent for a copy of the child's physical.

Standard #: 22VAC40-185-420-E-3
Description: Based on record review, the center failed to get at least annually parent confirmation that the required information in the child's record is up to date. Evidence: 1. Child E has been enrolled at the center since 9/28/2015 and the last annual review of emergency information from the parent was dated 08/03/2017.

Plan of Correction: I will ask the parent to review the child's information, make any needed changes and sign and date our form.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of five staff records, the center did not obtain a central registry report within 30 days of employment for a staff member.
Evidence:
1. Staff B (date of hire 6/25/2018) did not have a Virginia central registry finding in the staff file and has been employed more than 30 days.

Plan of Correction: I will send the VA central registry check.

Standard #: 63.2-1720.1-B-3
Description: Based on review of five staff records, the center did not obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years. Evidence: 1. Staff B (date of hire 6/25/2018) lived in another State in the past 5 years and did not have a record of obtaining a search of the central registry in that State. 2. Staff C (date of hire 3/12/2018) lived in another State in the past 5 years and did not have a record of obtaining a search of the central registry in that State. 3. Staff D (date of hire 2/12/2018) lived in another State in the past 5 years and did not have a record of obtaining a search of the central registry in that State.

Plan of Correction: I will send for the out of State central registry checks.
