Comments:
An unannounced 2 month follow-up from the initial inspection was completed today. The physical plant, 3 staff files, 5 children's files, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected between 11:40am -1:10pm. There were 77 children under the supervision of 17 staff. During the inspection, children were observed eating lunch, cleaning up for an resting during naptime, listening to storytime and enjoying free play. For lunch, the children were served hotdog with bun, broccoli, bananas and milk. The interactions between the children and staff were seen to be positive and constructive in nature. There are currently 4 staff certified in MAT (medication administration training). Should you have any questions, please contact me at mary.wizbicki@dss.virginia.gov.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on file review, 1 out of 5 children's records does not have the name address and phone number of 2 emergency contacts in case the parents cannot be reached. Evidence 1. Child #2 does not have an address for the 2nd emergency contact.

Plan of Correction: The Director sent an email to the parents requesting this during the inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observations, the center failed to maintain all areas of the center in a safe condition. Evidence 1. In the Octopus classroom, there was a red cushion to the back of a chair with holes and rips in it. 2. Outside on the playground area, the were two small areas (less than 6 inches wide and about 1 inch deep) where the resilient surfacing material had been pulled up.

Plan of Correction: A new cushion will be ordered. We will look into how to fix the areas on the playground.

Standard #: 22VAC40-185-560-F
Description: Based on observations, the posted menu did not indicate what was served to the children today. Evidence 1. The posted menu had the following listed to be served for lunch today, "chef's choice, seasoned mixed vegetables, fresh or chilled fruit and milk". What the children were served was hot dog on a bun, broccoli, bananas and milk.

Plan of Correction: Once the chef's choice has been determined, it will be added to the menu. This will be done prior to the day the food is served.
