Comments:
The inspector conducted an unannounced monitoring inspection from 10:15 a.m. until 12:20 p.m. There were 17 children in care with three staff providing supervision at all times. Children were observed finishing breakfast in the cafeteria, walking to the classroom, brushing their teeth, participating in circle time, going outside to play, and preparing for nap. Five children's records were reviewed. No medications were reported to be on site. The area of non-compliance is on the violation notice. Please call your inspector at 276-698-9981 with any questions.

Standard #: 22VAC40-185-330-B
Description: Based on observation and measurement, the center did not have the required fall zone around playground equipment. Evidence: 1. The inspector measured 4 feet from the red climbing dome to the fence on one side. The required use zone is 6 feet. 2. The inspector measured that the slide needs a use zone of 7 feet. The red climbing dome is in the use zone by approximately 2-3 feet.

Plan of Correction: The teacher will speak with the director about a long term solution. Staff will monitor children closely around the equipment.
