Technical Assistance:
Discussed with center management TB screenings, diapering and background checks. Also discussed to follow up with field print for staff still requiring fingerprints.

Comments:
An unannounced monitoring inspection was conducted today from 11:00am to 2:10pm. There were 70 children in care of 19 staff members. The children were observed playing with toys, a tunnel made out of boxes, playing outside, getting diapers changes and eating lunch. The lunch menu included chicken nuggets, plums, milk, whole wheat bread and peas. Medications and medication authorization records were reviewed. Emergency supplies were inspected. Violations:
Standard #: 22VAC40-185-240-D-1
Description: Based on staff interview, the center did not have staff that were trained to administer prescription medication. Evidence : 1. An emergency medication, Auvi-Q, was at the center for Child # 1. 2. The center has staff with the medication administration training (MAT). MAT staff did not have the additional training required to administer an Auvi Q.

Plan of Correction: We will get the training done.

Standard #: 22VAC40-185-270-A
Description: Based on observation, inside areas of the center were not maintained in a clean, safe and operable condition. Evidence: 1. Peeling paint was found on the wall behind the trash can in the preschool classroom, in the Toddler 1 classroom in the home living area, and in the K-Prep room by the home living area. 2. The plastic baseboard is separating from the wall in the K-Prep room by the home living area.

Plan of Correction: We will submit a work order to get these things fixed. For the time being we will cover the peeling paint with cardstock and put tape on the baseboard.

Standard #: 63.2-1720.1-B-3
Description: Based on record review and staff interview, center did not obtain out of state central registry findings for staff that have resided outside the state of Virginia within the last 5 years. Evidence: 1. Staff #2 was hired on 9/7/09. Staff # 2's records indicate that she resides outside of the state of Virginia. The out of state central registry was mailed for her on 1/17/18 but the findings were not obtained. Staff #1 stated that she did not conduct a follow up. 2. Staff # 3 was hired on 10/16/06. Staff # 3's records indicated that she resides outside of the state of Virginia. The out of state central registry was mailed for her on 2/20/18 but the findings were not obtained. Staff #1 stated that she did not conduct a follow up. 3. During the previous inspection in May 2018, Staff # 4's record indicated that she had resided outside the state of Virginia in the last five years. Staff # 4's out of state registry was mailed on 6/1/18. The findings were not obtained within 45 days and Staff # 1 stated that a follow up was not done to inquire about the results.

Plan of Correction: I will follow up with the agencies. If I do not get any definitive answers I will resubmit the forms.
