Comments:
An unannounced monitoring inspection was conducted from 8:00am-10:00am. Upon arrival there were 110 children in care with 21 staff present. A sample of 3 staff and 11 children's records were reviewed in addition to 5 medications. Children were observed during morning carpool, playing in learning centers and working one-on-one with a Spanish teacher.

Violations:
Standard #: 22VAC40-185-340-F
Description: Based on observation the center did not ensure children under 10 years-old were in actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom. Evidence A 4 year-old child #11 (dob12/12/13) was observed by the inspector and program director exiting the restroom in the bathroom hallway. Child #11 was sent from the big Pre-k classroom to the bathroom, located around the corner which was not in sight or sound supervision of staff. While in the restroom child #11 was not in sound supervision of staff.

Plan of Correction: The following corrective action was provided during the exit interview: The pre-k children from the big room will be sent to the smaller rooms with bathrooms to ensure the children are in sound supervision when using the restroom.

Standard #: 22VAC40-185-500-B
Description: Based on observation the center did not ensure all requirements were met when diapering. Evidence: In the 5 day three year-old classroom the changing table had a plastic bag of play-doh and children's artwork stored on the changing table.

Plan of Correction: The following corrective action was provided during the exit interview: The playdoh was discarded and the artwork moved. The director will remind staff that the changing table is not to be used for any activity other than changing diapers.
