The licensing inspector conducted an unannounced monitoring inspection on Wednesday, September 19, 2018 from approximately 8:45 am 12:45 pm to determine the center's compliance with licensing standards. During this inspection, there were 36 children present with 7 staff. The licensing inspector observed the children in care participating in a variety of activities to include eating their snack, listening to soft music, reading, working on alphabet sounds, vowels and consonant blends, and transitioning to chapel. The first aid kit and emergency supplies, posted emergency contacts, fire, and shelter-in-place drills were reviewed by the inspector. During this inspection, there were five children's records and five staff records reviewed. The center's classrooms, bathrooms, and kitchen areas were inspected. Standard #: 22VAC40-185-60-A Description: Based on review of children's record, the center did not ensure each child record contained the required information. Evidence: During the inspection on September 19, 2018, in one of five children's record, Child#3's (start date: 09/04/18) record was missing the name, address, and phone number of a second designated people to call in an emergency if a parent cannot be reacher. Plan of Correction: The center will request second contact person from the parent.
Standard #: 22VAC40-191-60-C-1 Description: Based on review of staff record, the center did not ensure all staff have an original criminal history record report within 30 days or employment or volunteer service. Evidence: During the inspection on September 19, 2018, one of five staff records were reviewed and in Staff#2's record the criminal history report was documented mailed on 01/25/18, however, the results of the criminal history record was not found. Plan of Correction: The center will contact OBI to ensure the staff complete the required background check and all staff will have OBI fingerprinting completed by September 30, 2018
