Comments:
An unannounced monitoring inspection was conducted from 7:30am through 8:30am. There were 18 children in care ranging in age from four years old through 10 years old with two staff. Children and staff emergency contact information was reviewed. Children were observed in teacher and self-directed activities. Required postings, first aid kit, emergency items and documentation of insurance were reviewed. Violations regarding immunizations, first aid kit, menus and outlet covers were identified and appear on the violation notice.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review and interview the center failed to ensure that each child has received the immunizations required by the State Board of Health before the child can attend the center. Evidence: 1 - The record for child 1 does not contain documentation of any immunizations. a. Child 1 was present for care. b. Child 1 has first date of attendance as 9/5/2018. 2 - Staff 1 confirmed that there is not an immunization record in the file of child 1.

Plan of Correction: Immunization documentation will be obtained from the parents as soon as possible.

Standard #: 22VAC40-185-290-3
Description: Based on observation and interview the center failed to ensure that electrical outlets have protective covers. Evidence: 1 - There are seven electrical outlets that do not have protective covers in the child room and the rest rooms used by children in care. 2 - Staff 2 confirmed that there are electrical outlets that do not have the required protective covers.

Plan of Correction: Going forward center management will complete a safety check of the child care room and rest rooms and replace any missing electrical outlet covers before children arrive for care.

Standard #: 22VAC40-185-540-C
Description: Based on equipment review and interview the center failed to maintain a first aid kit with all required items. Evidence: 1 - The first aid kit provided does not contain scissors or tweezers. 2 - Staff 1 stated that she could not locate the scissors or tweezers.

Plan of Correction: Going forward the first aid kit will be checked and missing items replaced on a monthly basis.

Standard #: 22VAC40-185-560-F
Description: Based on document review and interview the center failed to ensure that a current one week menu is posted in the center. Evidence: 1 - There is no dinner menu posted in the center. 2 - Staff 1 confirmed that dinner is served each evening at the center and the dinner menu is not posted.

Plan of Correction: Going forward center management will obtain the current menu from the cafeteria staff and post it on the center's bulletin board. The posting will be show at least the current week.
