An announced record review inspection for PCCA Refuge Head Start #1 was completed on 09/20/2018. The inspector reviewed 2 staff record during the inspection today. The inspector reminded the Director of Human Resources of the September 30, 2018 deadline for all staff to be fingerprinted. Standard #: 22VAC40-185-70-A Description: Based on record review and discussion with staff, the facility failed to ensure that all staff records contain all components are required by the standards. Evidence: The record for Staff #2 did not contain the following information: date of employment, documentation that two or more references as to character and reputation as well as competency were checked before employment, the record only contained one reference check, and written information to demonstrate that the individual has completed orientation training required by the job position. Plan of Correction: The date of employment and the written verification of orientation were added to the record during the inspection; a second reference will be obtained.
