Consultation provided regarding organization of staff and children records. Please be sure that any curriculum posted is current and available to parents.

Comments:
An unannounced monitoring inspection was conducted this date. A total of 97 children with 14 staff present today. Observation and staff interviews conducted in all 8 classrooms. Transition from lunch to diapering to naptime was observed. A review of both staff and children records was made. Some violations found today. Thank you for your cooperation today. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on child record review, one physical exam was not found for one child. Evidence: A physical exam was not found for Child #3.

Plan of Correction: Child did not have physical form in file. Contacted parent's on 9/19/18 stating we need physical form as soon as possible. Child has a 9 month checkup the first week of October, parents will submit paperwork then.

Standard #: 22VAC40-185-270-A
Description: Based on observation of all 8 classrooms, some areas of the center was not maintained in a clean, safe and operable condition. Evidence: The following areas were not maintained: l. The base molding located in the entry of the beginners classroom is not attached adequately, presenting a tripping hazard. 2. The vent in the beginner's classroom was found rusted and dirty. 3. The blinds in the beginner's classroom was found dirty with debris. 4. Peeling paint was found in the beginner's classroom by the bathroom sinks. 5. Two toilet seats located in the bathroom of the beginner's classroom was not attached appropriately presenting a potential injury. 6. The base molding located in the toddler classroom was not attached adequately exposing debris accessible to the children in care. 7. Trash can located in the toddler bathroom was found overflowing with pull ups and the trash bag was not adequately lined to prevent spills to the trash can.

Plan of Correction: 1. Tripping hazard in Beginner classroom was fixed by maintenance and no longer a tripping hazard on 9/21/18. 2. The vent in the beginner classroom was removed and cleaned and spray painted by maintenance on 9/21/18 3. Blinds in Beginners have been scrubbed down. Contacted company to replace with new blinds on 9/25/18. 4. Peeling paint found in the bathroom by the sink. Work order has been place to paint the bathroom sink area. 5. Toilet seats in the beginner classroom has been tighten and no longer is a hazard for children. 6. Molding in the toddler class has been reattached and is no longer accessible to children. 7. Trash can in toddler was found with pull ups. Spoke with teachers about using the proper trash can to dispose of pull ups. Staff training on 10/4/18 will go over the importance of the proper way to dispose of soiled diapers.

Standard #: 22VAC40-185-280-B
Description: Based on observation of the physical plant, hazardous substances were found unlocked and accessible to children. Evidence: Cleaning agents were found in the cabinet under the sink in the beginner's classroom.

Plan of Correction: Cleaning agents were secured immediately.

Standard #: 22VAC40-185-500-A
Description: Based on observation of the Infant 2 and Toddler classroom, hand washing was not completed by staff. Evidence: The toddler classroom had one staff on duty who was observed changing children's diapers. Staff did not wash hands after diapering a child and proceeded to diaper another child. In the Infant 2 classroom one staff did not wash hands after diapering a child and proceeded to diaper another child.

Plan of Correction: Hand washing was not completed by staff during the diapering procedure. Staff training on 10/4/18 will include proper hand washing and diapering procedures.

Standard #: 22VAC40-185-500-B
Description: Based on observation of the Toddler Classroom, the diaper surface used was not cleaned and sanitized after diapering children. Evidence: The toddler classroom had one staff on duty who was observed diapering children. Staff failed to clean the diapering surface with soap and water and sanitize with bleach and water after each child's diaper change.

Plan of Correction: Staff in toddler classroom failed to sanitize in between diaper changes. Staff training on 10/4/18 will include proper sanitizing procedure between diaper changes.
