Technical Assistance:

An unannounced monitoring inspection was conducted on Tuesday, September 18, 2018 from 9:30am to 11:40am to determine the center's compliance with licensing standards. The center's director assisted the inspector throughout this inspection. The census for today's inspection consisted of 16 children with 2 staff and 1 volunteer in the licensed area of the center. During the inspection, the licensing inspector observed the children during circle time, eating snack, taking bathroom breaks, and going outside. The staff were observed having positive interactions with the children. The most recent evacuation drill was documented on 09/05/2018 and two shelter-in-place drills were documented on 05/01/2018 and on 09/17/2018. Standard #: 22VAC40-185-130-A
Description: Based on a review of five children's records on 09/18/2018, the center did not obtain documentation that one child had been adequately immunized before the child attended the center. EVIDENCE: 1) The record for Child #2 (DOA: 09/04/2018) did not contain an immunization report.

Plan of Correction: Corrected during inspection. The Director contacted the parent and the immunization report was received via email on 09/18/2018.

Standard #: 22VAC40-185-70-A
Description: Based on a review of staff records and interview on 09/18/2018, the center did not obtain all required information for one out of two staff records reviewed. EVIDENCE: 1) The record of Staff #1 (rehired on 09/01/2018) did not have documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering. 2) During interview, the center's Director stated two references checks had not been obtained for Staff #1 before Staff #1 was rehired. The Director stated previous reference checks had been completed in the past, but they were not available for review on 09/18/2018.

Plan of Correction: Per the Director: Will find previous records and submit via email on 09/30/2018.

Standard #: 22VAC40-185-260-B
Description: Based on record review and interview on 09/18/2018, the center did not obtain an annual health inspection report. EVIDENCE: 1) Record review on 09/18/2018 indicates the most recent health inspection was completed on 02/27/2017. 2) During interview on 09/18/2018, the center's Director confirmed the annual health inspection had not been obtained and the local agency had not been contacted.

Plan of Correction: Per the Director: We will have this completed ASAP.
