Comments:
A Subsidy Health and Safety inspection was conducted on 09/28/2018 to review supplemental health and safety requirements. An unannounced monitoring inspection was conducted today from approximately 9:45am-1:35pm. There were 61 children in the direct care and supervision of 9 staff. A sample of 6 children?s records, 9 staff records and 3 medications were reviewed. on the playground, eating lunch, during choice time, reading and napping. 3 staff are new to this location since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the administrative teacher. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: VENDSUB-000-012-B
Description: Based on record review, all staff did not complete the Virginia Preservice Training. Evidence: Staff A, B and C did not have evidence that they've completed the Virginia Preservice Training.

Plan of Correction: Preservice trainings are kept at the main office. I will ask them to e-mail them over by the end of the day.

Standard #: VENDSUB-000-012-E-1
Description: Based on record review, all staff did not have a current CPR certification. Evidence: Staff D did not have proof that CPR training was completed.

Plan of Correction: Staff D is a lunch floater. She will carry the required trainings.

Standard #: VENDSUB-000-012-E-2
Description: Based on record review, all staff did not have a current first aid certificate. Evidence: Staff D and E did not have a current first aid certificate.

Plan of Correction: Staff D and E will get those by the end of the day.
