Technical Assistance:
Licensing inspector discussed expiring medication authorization with staff.

Comments:
An unannounced monitoring inspection was conducted today from approximately 9:45am-12:45pm. There were 61 children in the direct care and supervision of 9 staff. A sample of 6 children?s records, 9 staff records and 3 medications were reviewed. on the playground, eating lunch, during choice time, reading and napping. 3 staff are new to this location since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the administrative teacher. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records were incomplete. Evidence: Child A was missing the parental agreements.

Plan of Correction: Mom was sick, I will ask her to return the required documents.

Standard #: 22VAC40-185-70-A
Description: Based on record review, staff records were incomplete. Evidence: 1 of 9 staff records were missing required information. Staff C was missing an address and emergency contact information.

Plan of Correction: Staff A will complete an emergency contact form today.

Standard #: 22VAC40-185-550-D
Description: Based on record review and interview with staff, evacuation drills were not practiced monthly. Evidence: An evacuation drill was not practiced for the month of June 2018. Staff B stated that the school year operated from September to June.

Plan of Correction: We will conduct evacuation drills monthly. We have conducted two for September.
