An unannounced monitoring inspection was conducted on October 2, 2018 during the before care portion of the program. There were 18 children present at arrival to the facility with two staff members. The children were observed arriving to the program, playing with various tabletop activities to include blocks, coloring, puppets and listening to music. The licensing inspector reviewed the physical plant, supervision, required postings, children's records, emergency evacuation and shelter-in-place documentation, and nutrition. The posted menu indicated that today's snack would consist of yogurt and bananas. Three medications were reviewed. Five children's records were reviewed during the inspection. The exit interview was conducted with the Site Director.

Standard #: 22VAC40-185-60-A
Description: Based on record review, in one of five children's records reviewed, the licensee did not ensure that each child's record was documented with all required information. Evidence: During the inspection conducted on October 2, 2018, the following information had not been documented in each child's record: Child #5-a complete address for two people to contact in the event of an emergency where the parent cannot be reached and the work phone numbers for both parents.

Plan of Correction: Site Director will speak with the parents and get them to update the child's record.

Standard #: 22VAC40-185-260-A
Description: Based on review, the center did not provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: During the inspection conducted on October 2, 2018, the annual fire inspection report for the facility was more than a year old and was dated 4/16/17.

Plan of Correction: Site Director will get the form and put in the binder today.

Standard #: 22VAC40-185-550-D
Description: Based on observation and inspection of the facility, the licensee did not ensure that the center implemented a monthly practice evacuation drill. Evidence: During the inspection conducted on October 2, 2018, there was no documentation of an evacuation drill practiced in September 2018.

Plan of Correction: Site Director will make sure that they have the forms and that it is written down. A drill was completed in September.
