22VAC40-185 STAFF QUALIFICATIONS AND TRAINING.
22VAC40-185 STAFFING AND SUPERVISION.
On 8/15/2018 an incident regarding the care of children was self-reported to the licensing office. Interviews were conducted and documentation was reviewed on 9/6/2018 and 9/12/2018. Based on the preponderance of evidence gathered, the were no violations regarding the self-reported incident, however separate violations were found. Standard #: 22VAC40-185-70-A Description: Based on record review, documentation of one staff person's reference checks did not contain all required components. Evidence: Staff #3's phone reference check did not contain the name of the person that was contacted. Plan of Correction: Moving forward, names will be included on the reference sheets.
Standard #: 22VAC40-185-240-A Description: Based on record review and interview, one staff member did not complete the Recognizing Child Abuse and Neglect and the legal requirements for reporting suspected child abuse as required by section 63.2-1509 of the Code of Virginia. Evidence: Staff #3 (date of hire: 4/13/2018) file did not contain the Child Abuse and Neglect training. Center management has confirmed that this staff member did not complete the training. Plan of Correction: All staff will complete this training before they begin.
Standard #: 22VAC40-185-340-D Description: Based on interview, one classroom did not have at least one staff member who met the qualifications of a program leader who was regularly present. Evidence: When discussing the staffing of the Pre-kindergarten classroom, Staff #1stated that she was a floater and is not permanently located in this classroom. Staff #2 stated that she was the assistant of the classroom. Center management confirmed that Staff #3 did not meet the qualifications of a program lead. Center management stated that Staff #3 was supposed to be completing their CDA credential and it should have been completed in July 2018, however the CDA training was not completed. Center management stated that they would often be "in and out" of the classroom along with Staff #4 who was often be "in and out" to monitor the classroom. There was no regularly present staff member who met the program leader qualifications. Plan of Correction: We have established a programmatic lead teacher for this classroom.
