An unannounced monitoring inspection was conducted on October 3, 2018, from approximately 3:52pm - 5:25pm . There were 42 children present with three staff members. The children were observed participating in a variety of activities in the cafeteria, a tile word game, reading, homework and eating snack. The licensing inspector reviewed five children records.
Standard #: 22VAC40-185-130-A Description: Based on 1 of 5 records reviewed, the licensee did not ensure the records contained a immunization before the child can attend. Evidence: During the inspection conducted on October 3, 2018, the record for child #1 ( start date 09/04/2018) did not have a immunization record before the child attended. The staff confirmed the immunization record has been requested by the center. Plan of Correction: Corrected while inspector present.
Standard #: 22VAC40-185-260-A Description: Based on record review, the licensee did not ensure the fire inspection was done annually. Evidence: During the inspection conducted on October 3, 2018, the documentation states the most recent fire inspection was conducted on August 29, 2017. Staff #1 contacted the school on October 1, 2018. The school did not have document of more recent fire inspection. Plan of Correction: The center director will follow up with the office.
Standard #: 22VAC40-185-550-C Description: Based on observation and interviews, the licensee did not ensure there was a map that includes shelter in place location posted in the facility. Evidence: During the inspection conducted on October 3, 218, the shelter in place locations were not included on a posted map in the building and evacuation and shelter in place procedures were not posted. Confirmed with staff. Plan of Correction: Director will make changes to the map and post evacuation and shelter procedures.
Standard #: 22VAC40-185-560-F Description: Based on observation, the licensee did not ensure children were allowed second helpings for snack. Evidence: During the inspection conducted on October 3, 2018, two children asked to have more snack. Staff #1 said "no you can only have one" and " no just one." Two children did not receive a second helping. Interviewed staff stated for seconds crackers are usually given. Plan of Correction: The program director will contact field coach for further guidance on second helpings.
