An unannounced monitoring inspection was completed on September 24, 2018 from approximately 9:45a.m. to 12:15p.m. Fifty-five children were in care during the inspection. Staff and children, ages two to four years old, were observed in five classrooms with adequate ratios, engaging in activities to include circle, snack and free center time and hand washing and toileting. Children?s equipment, learning materials, outdoor and classroom space, bathrooms and the center?s first aid and emergency supplies were inspected. Five children?s records, five staff records, annual fire and health inspections, fire drill and shelter in place logs, medication authorizations and logs, medication administration and daily health observation certifications, injury reports, daily attendance and required postings were reviewed. The center does not provide transportation. The Director was available for interview, inspection and present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. There were five citations for violations of the Standards for Child Day Centers. See the violation notice on the Department?s public web site for violations of the Standards.
Standard #: 22VAC40-185-60-A Description: Based on a review of children?s records, the licensee failed to maintain all the elements of a child?s record for one of five children. Evidence: 1. Child #1 was enrolled at the center on September 10, 2018. 2. Child #1?s record lacked a custodial parent's work number and the child's physician's number. 3. Administrator #1 acknowledged this information was not documented. Plan of Correction: Director will obtain missing information from the parent.
Standard #: 22VAC40-185-70-A Description: Based on a review of staff records, the licensee failed to maintain all the elements of staff?s records for three of five staff. Evidence: 1. Staff #1 was hired on January 2, 2017, Staff #2 on September 27, 2016 and Staff #5 on September 5, 2017. 2. Staff #1, Staff #2 and Staff #5's records lacked documentation of age verification. 3. Administrator #1 acknowledged this information was not documented in these staff records. Plan of Correction: Director will obtain copies of staff identification for the records.
Standard #: 22VAC40-185-270-A Description: Based on observation of outdoor space, the licensee failed to ensure that areas and equipment of the center are maintained in a clean, safe and operable condition. Evidence: 1. A brown plastic log cabin and a red and white ride on fire truck were observed on the playground and accessible to children. Both contained areas in which the plastic was broken, creating sharp and jagged edges. 2. Administrator #1 acknowledged these pieces of play equipment were broken. Plan of Correction: Director will fix the broken pieces of playground equipment/toys.
Standard #: 22VAC40-185-510-D Description: Based on a review of medication authorizations, licensee failed to maintain available medical authorizations for medication during the entire time it is effective for two of four children. Evidence: 1. A prescription epi-pen and over the counter benadryl was observed for Child #6 and over the counter benadryl was observed for Child #9. 2. Medication authorizations were not present for these medications. 3. Administrator #1 acknowledged the center did not have authorizations for these medications. Plan of Correction: Director will obtain the required authorizations for these medications.
Standard #: 22VAC40-185-550-D Description: Based on a review of records, the licensee failed to implement monthly practice evacuation drills and a minimum of two shelter-in-place practice drills for the most likely to occur scenarios.
Evidence: 1. Monthly fire drill and shelter in place logs were observed. 2. The center completed only one shelter in place drill for 2017, dated December 8, 2017.
Plan of Correction: Cannot be corrected. The Director will ensure that at least two shelter in place drills are completed annually.
