Comments:
Thank you for your time and assistance with the unannounced renewal inspection conducted today, October 15, 2018. The on-site visit was from 10:15 a.m. to 2:45 p.m. On this date, there were 13 children present in the four-year-old morning class and 10 children present in the afternoon three-year-old class. A total of two staff members were present during each class. The total enrollment for all three classes was 43 children. The exception to compliance was noted on the violation notice and the risk rating for the violation was noted on the supplemental page. There were five staff members' records reviewed and five children's records reviewed. Three staff members and four children were interviewed. Two classrooms were observed. The medication and medication paperwork were reviewed for three children. All snacks were provided by the parents. Due to inclement weather, outdoor play could not be observed. The theme for the week was Shapes and Colors. The children enjoyed making owls out of shapes. Some of the items reviewed and observed included: annual fire and health inspections, emergency preparedness plan, injury prevention plan, first aid kit, emergency supplies, warm water temperature, choice time, circle time, story time, music and rhythm activities, arrival and dismissal procedures, art activities, table top activities, small motor activities, indoor large motor play, snack, hand washing, and bathroom procedures. Standard #: 22VAC40-185-510-G
Description: Based on the review of medication and medication paperwork for three children and verification from staff, the center failed to have written parent authorization to administer the medication for one child. Evidence: 1. There was emergency medication on-site for Child B but there was no written parent authorization which would allow the center to administer the medication if needed.

Plan of Correction: The staff spoke with the parent of the child during the inspection and will obtain authorization as soon as possible.
