22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was made on 10/16/18. There were 32 children present in two classrooms with four staff supervising. Five children's records and one as needed medication were reviewed. Four staff records were reviewed on 10/17/18 and the inspection was completed in person on that date. The inspector arrived at the school on 10/16/18 at 8:30 am and departed at 10:30 am. The children were observed during breakfast, during free time in the classrooms, during large group times, and during story time. There was discussion with the teacher about the following: practicing and documenting an October fire drill, practices for independent contractors (speech therapist from the public school) to work with children with IEPs alone and unsupervised.
Standard #: 22VAC40-185-60-A Description: Based on record review, the center failed to ensure that all required information was in each child's record. Evidence: 1. The record of child #2 was missing one of two required emergency contacts for the child. Two emergency contact names, addresses, and phone numbers are required for each child's record. Plan of Correction: The teacher will talk with the parent to get documentation of a second emergency contact.
Standard #: 22VAC40-185-260-A Description: Based on observation and interview, the center failed to ensure that there was an annual fire inspection report. Evidence: 1. The posted fire inspection was observed to have a date of completion of 2/23/17. 2. The teacher stated that there has not been another fire inspection completed since that date. Plan of Correction: The transportation office manager will contact the fire marshal and school to see if the inspection has been completed. The inspection form will be posted once it is completed.
