22VAC40-191 Background Checks (22VAC40-191)
An unannounced inspection was completed on 10/16/2018 from 9:30 A.M.-1:30 P.M. At the time of the inspection 45 children were in care with eleven staff and one administrators present. Five children records, five staff records, six injury reports, emergency plan, first aid kits, evacuation maps, emergency supplies, playground, and posted required information were reviewed. Children were observed during infant feeding, diaper chage, infant floor time, handwashing, center play, fine motor manipulative activity, story and song time, lunchtime, behavior guidance, and naptime. Standard #: 22VAC40-185-270-A Description: Based on observation, the center failed to maintain all materials in a clean, safe, and operable condition. Evidence: 1. In the Four Year Old Classroom on the door that leads to the playground the blinds were broken and had busted slats that were projecting out and into a the path that a child may walk. 2. Staff verified that the blinds were broken and there were busted slats projecting out. Plan of Correction: The administrator will purchase new blinds this week and have them hung, and in the future will monitor the blinds and have them replaced before they become a safety issue.
Standard #: 22VAC40-185-280-B Description: Based on observation, the center failed to ensure that all hazardous materials were kept in a locked location. Evidence: 1. In the Four Year Old Classroom there were four cans of Equate Shaving Cream sitting on the art shelve that was accessible to children. 2. The warning label on the cans stated "keep out of the reach of children", and "inhaling could be harmful and fatal". 3. Staff verified that the cans of Equate Shaving Cream were located on the shelve and accessible to children. Plan of Correction: Staff removed the cans from the art shelve and put them in a locked location. Administrator will review policies and standards regarding hazardous materials.
Standard #: 22VAC40-185-290-3 Description: Based on observation, the center failed to ensure that all electrical outlets had protective covers. Evidence: 1. In the Four Year Old Classroom there was a power strip under the computer desk that did not have protective covers in two electrical outlets. 2. Staff verified that the electrical outlets did not have protective covers. Plan of Correction: Staff immediately went and located protective covers to place in the electrical outlets.
Standard #: 22VAC40-185-510-J Description: Based on observation, the center failed to ensure that all medication was kept in a locked location and inaccessible to children. Evidence: 1. In the Infant Classroom there was a bottle Children's Advil in the side pocket of a diaper bag. 2. The bag was hanging on a hook below the cubbies and was accessible to children. 3. Staff verified that the medication was in the diaper bag and was not in a locked location. Plan of Correction: Staff immediately removed the medication from the diaper bag and placed in a locked location. The Administrator will review with staff the medication policy and to check diaper bags daily, and will send a memo out to parents reminding them of the centers policy on medication.
Standard #: 22VAC40-185-550-M Description: Based on review of injury reports, the center failed to document all required information on three out of six injury reports reviewed. Evidence: 1. Two out of six injury reports were missing the time the parent was notified of injury. 2. One out of six injury reports was missing the method of notification. 3. Two out of six injury reports were missing the section requiring any future action to prevent recurrence of the injury. Plan of Correction: The administrator will complete a staff training with all staff on the requirements for injury reports and how to correctly fill them out.
