Comments:
An unannounced renewal inspection was conducted with the director from 10:25am and ending at 1:15pm. An exit meeting was conducted with the director from 4:00- 4:15PM. There were 26 children in care during the inspection. The children's ages ranged between 7 months and 4 years old. The children were observed playing outside and hand watering the garden. Younger children were observed playing with blocks and soft toys. The pavilion and large playground area were unable to be inspected due to inclement weather conditions. Six children's records and five staff records were reviewed. One medication was reviewed and in compliance. The first aid kit, flashlight, and weather band radio were observed and in compliance. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on a review of six children's records, the center did not have documentation of physical in each child's file prior to or within 30 days of first attendance. Evidence: Child A (start date 4/19/2018) and Child B's (start date 1/20/2017) records did not have documentation of a physical.

Plan of Correction: The director contacted the parents during the inspection. Child A's physical was sent by email that day. Child B is out of school until their physical is obtained.

Standard #: 22VAC40-185-80-A
Description: Based on a review of the written attendance record and interview with staff the center did not ensure all children were included on the written attendance record. Evidence: There were eight children present in the preschool classroom and seven signed in on the attendance sheet.

Plan of Correction: The teacher signed in the child that was missing from the attendance sheet. Staff was reminded to make sure that all children are signed in and out.

Standard #: 22VAC40-185-260-B
Description: Based on a review of documentation, the center did not have documentation of an annual approval from the health department as required. Evidence: The most recent annual health inspection was documented as 9/01/2017.

Plan of Correction: The director has spoken to the Health Department and the health inspector is expected to come.

Standard #: 22VAC40-185-270-A
Description: Based on observation not all of the areas and equipment of the center were maintained in a safe condition. Evidence: On the infant playground there was a plastic sandbox container filled with standing rainwater and debris. On the preschool playground, the regular-size picnic table had a nail head protruding from the bench seat and a child-size picnic table had a screw protruding from the bench seat, creating a scratching hazard.

Plan of Correction: The director emptied the water from the plastic sandbox container during the inspection. The sandbox will be removed from the center. The nail will be removed and replaced with a screw. The screw will be fixed.
