Discussed completion of staff records with program Director. LI must review completion of missing/incomplete background checks within 10 business days.
Today an unannounced focused Monitoring Inspection took place between the hours of 2:45pm and 4:45pm. At the start of the inspection there were a total of 67 children with 6 staff; all ratios were incompliance. A three staff files were reviewed. The children were observed unloading the programs mini-bus, eating snack, splitting up into groups to play card games, drawing, doing homework and playing ball games in the gym. Standard #: 22VAC40-185-160-A Description: Based on record review and interview, one staff member did not have documentation of a Tuberculosis (TB) test on file. Evidence: Staff #5 file does not contain documentation of a TB test. The program Director stated that a test was completed in June of 2018 but there is no documentation available stating a positive or negative result. Plan of Correction: I think I have the documentation filed away. If I can't find it I'll contact the nurse that completed the test to obtain additional documentation.
Standard #: 22VAC40-185-70-A Description: Based on a review of records and interview, a record was not kept for each staff person. Evidence: Staff #2 (date of hire: 10/15/2018) does not have a staff record. The program director has confirmed that this staff person started this week and no paperwork has been completed yet. Plan of Correction: I've already let this staff member know we need to get her background checks. I'll get the rest of the file completed next week.
Standard #: 22VAC40-191-60-B Description: Based on record review and interview, the program has not obtained a sworn disclosure statement for one employee. Evidence: Staff #2 (date of hire: 10/15/2018) file does not contain a sworn disclosure statement. The program Director has confirmed that a sworn disclosure statement has not been completed. Plan of Correction: We'll get this done today.
Standard #: 63.2-1720.1-B-2 Description: Based on record review and interview, the program has not obtained fingerprint results (eligibility letter), prior to being permitted to work with children, that states whether or not an employee is eligible to work with children. Evidence: Staff #2 file does not contain an eligibility letter. The program Director has confirmed that this staff member has not completed their fingerprinting yet. Staff #2 was observed working with the children at today's inspection. Plan of Correction: I'll have her make an appointment to get this done.
