Discussed with the center director that the gate leading to the air conditioning/heating unit is to remain closed at all times.
An unannounced renewal inspection was conducted today from 11:50am-3:45pm. There were 77 children (infants-4yrs.) directly supervised by 15 staff. The physical plant, 7 staff records, 7 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to a story, being bottle fed, napping, and eating lunch. Today's lunch consisted of diced chicken, whole-grain flour tortilla, corn, peas and carrots and milk. Diapering and hand-washing procedures were also observed. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-185-270-A Description: Based on record review, areas and equipment of the center were not maintained in a safe and operable condition. In the preschool classroom, two small holes were observed on the wall next to the boys bathroom that could present a finger entrapment. In the toddler classroom a small area rug is beginning to fray on one of the edges creating a tripping hazard. Plan of Correction: We will put a work order to get those items fixed.
Standard #: 22VAC40-185-550-M Description: Based on review of children's serious and minor injuries, injury reports did not have complete documentation of required information. Evidence: Three out of six injury reports reviewed were missing the time of parent notification and documentation of how parents were notified. Plan of Correction: We will send a memo this week to retrain them on incident and accident reports, I will talk about it at our next staff meeting
