Comments:
An unannounced monitoring inspection was conducted on October 23, 2018. Upon arrival to the facility, during the aftercare portion of the program, there were two staff present with 24 school age children. The children were observed transitioning to the program, doing various table top activities to include playing with blocks, doing homework, playing with Legos and doing homework. The children were also observed having snack which consisted of a fruit cup and crackers. Five children's records were reviewed on this date. Four medications were reviewed. Licensing Inspector reviewed supervision, activities, equipment, nutrition, children's records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports.

Standard #: 22VAC40-185-260-A
Description: Based on observation and inspection of the facility, the licensee did not provide to the licensing representative an annual fire inspection report from teh appropriate fire official having jurisdiction. Evidence: During the inspection conducted on October 23, 2018, the fire inspection report for the facility was more than a year old and was dated 10/03/2017.

Plan of Correction: The school did not provide a copy of the fire inspection but hopefully we will have it in the near foreseeable future.
