Comments:
An unannounced monitoring inspection was completed on 10/23/2018. There were 37 children, ages 5 years-12 years, and 5 staff providing direct care and supervision present during today's inspection. The inspector reviewed 5 children's records during today's inspection. The staff records will be viewed on a later date at the Corporate Office. There is no medication being administered at this time. The children were observed in the following activities: during a fire drill, having afternoon snack, during outdoor free play, participating in a special presentation by a guest, and during homework time. The inspector reviewed the emergency procedures and emergency supplies during the inspection today. The inspector discussed the following with the site staff: children's belongings, attendance, children's allergies, sanitizing, and changes to the subsidy requirements. The inspector arrived for the inspection at 3:00 p.m. and departed at 5:45 p.m. Thank you for your time today. If you have any questions, please contact Becky Forestier at 540-309-2835.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the facility failed to ensure that all children's records contain all components as required by the standards. Evidence: 1. The record for Child #1 did not contain the names of persons authorized to pick up the child. 2. The record for Child #2 did not contain the following information: the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached; the addresses for both people listed were incomplete, the names of persons authorized to pick up the child and the health information as required by 22VAC40-185-140. 3. The record for Child #3 did not contain the following information: the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached; the addresses for both people listed were incomplete, the names of persons authorized to pick up the child and the health information as required by 22VAC40-185-130 and 22VAC40-185-140. 4. The record for Child #4 did not contain the following information: the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached; the addresses for both people listed were incomplete, the names of persons authorized to pick up the child and the health information as required by 22VAC40-185-140. 5. The record for Child #5 did not contain the following information: the names of persons authorized to pick up the child and the health information as required by 22VAC40-185-130 and 22VAC40-185-140.

Plan of Correction: The Program Director will contact the Administrative Office to have them assist in getting the information as well as to speak to the parents.

Standard #: 22VAC40-185-260-A
Description: Based on observation and discussion with the staff, the facility failed to obtain an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: There was no annual fire inspection report from the appropriate fire official having jurisdiction available to review during the inspection.

Plan of Correction: The Site Director will ask the office for a copy of the latest fire inspection.

Standard #: 22VAC40-185-280-E
Description: Based on observations and discussion with the staff, the facility failed to ensure that cleaning materials and insecticides/pesticides shall be stored in areas physically separate from each other. Evidence: There was an aerosol spray bottle of Kill Zone Flying Insect Killer on the same shelf of the plastic storage cabinet as the cleaning materials.

Plan of Correction: The insecticide/pesticide was disposed of during the inspection.

Standard #: 22VAC40-185-280-G
Description: Based on observations and discussion with the staff, the facility failed to ensure that if hazardous substances are not kept in original containers, the substitute containers shall clearly indicate their contents. Evidence: During the cleaning of the tables following afternoon snack, Staff #5 was observed holding a clear plastic spray bottle containing a light blue liquid solution that was not labeled; neither Staff #2 nor Staff #5 were aware of the contents of the bottle, only that is was used for cleaning tables.

Plan of Correction: The bottle will be labeled.
