22VAC40-191 Background Checks (22VAC40-191)
The licensing inspector conducted an unannounced renewal inspection on 10/23/2018 from 10:38am to 3:00pm. The children were observed playing on the playground, eating lunch and napping. A formal observation was completed with the infant and three year old classrooms. Lunch was provided by each child's parent. The morning and afternoon snack menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today as well as the van. Five children?s records and five employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. Six injury reports were reviewed as well as the center?s injury prevention plan which was updated 10/1/2018. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 10/19/2018 Last shelter-in-place drill: 09/17/2018 Last fire inspection: 09/10/2018 Last health inspection: 7/11/2018 Today, the following child to staff ratios were observed: Infants - 6:3 Toddlers ? 8:2 Twos - 8:2 Threes - 12:2 Fours ? 18:3 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-240-A Description: Based on record review, the center did not ensure that one of five staff records contained documentation of the following training by the end of their first day; recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse as required by 63.2- 1509 of the Code of Virginia. Evidence: The record of staff #4 (start date: 10/22/2018) did not contain documentation of child abuse and neglect training. Plan of Correction: Staff will get the training done by tomorrow.
Standard #: 22VAC40-185-270-A Description: Based on observation, the center did not ensure that areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: The inspector observed areas of splintering and sharp wood on the fence surrounding the playground. The inspector also observed a children's chair in the Pre-K classroom with two tears in which the foam was exposed. Plan of Correction: Maintenance staff will take care of sanding fence and replacing any boards that need to be replaced. I will remove the chair and purchase a new one for the reading center.
Standard #: 63.2-1720.1-B-2 Description: Based on record review, the center did not obtain fingerprint check results for two of five staff prior to the first day of employment. Evidence: 1. The record of staff #4 (start date: 10/22/2018) did not contain documentation of fingerprint check results. 2. The record of staff #5 (start date: 10/23/2018) did not contain documentation of fingerprint check results. Plan of Correction: Both of the staff had their fingerprints done today and from now on all of the staff will have theirs before hire.
