Comments:
An unannounced monitoring inspection was conducted on October 30, 2018. Upon arrival to the facility, there were 38 children with seven staff members. Additional staff were on site for administrative and support purposes. Children were observed arriving to the center, playing with blocks and puzzles in centers, and during outdoor play. Licensing Inspector reviewed supervision, activities, equipment, indoor and outdoor areas, diapering and handwashing procedures, medication, children and staff records, emergency evacuation and shelter-in-place drill. Five children's records and seven staff records were reviewed. Two medications were reviewed.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review, in one of seven staff records reviewed, the licensee did not ensure that each staff member and individual from an independent contractor shall submit documentation of a negative tuberculosis (TB) screening. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing or volunteering. Evidence: During the inspection conducted on October 30, 2018, the hire date for Staff #6 was 10/1/18 and the facility accepted a TB screening more than 12 months old that was dated 8/15/17.

Plan of Correction: Director will contact the health department and have her to make an appointment.

Standard #: 22VAC40-185-70-A
Description: Based on record review, in one of seven staff records reviewed, the licensee did not ensure that each staff record was maintained with all required information. Evidence: During the inspection conducted on October 30, 2018, there was no documentation that two or more references as to character and reputation as well as competency were checked before employment for Staff #5.

Plan of Correction: Director will make contact with the references by tomorrow.

Standard #: 22VAC40-185-380-A
Description: Based on observation and inspection of the facility, the licensee did not ensure that there shall be a posted daily schedule that allows for flexibility as children's needs require. Evidence: During the inspection conducted on Octyober 30, 2018, there was not a posted daily schedule in the Oaks 1 and Oaks 2 classrooms.

Plan of Correction: The schedule will be posted.

Standard #: 22VAC40-185-440-J
Description: Based on observation and inspection of the facility, the licensee did not ensure that there shall be at least thirty inches of space between service sides of occupied cribs and other furniture where that space is the walkway for staff to gain access to any occupied crib. Evidence: During the inspection conducted on October 30, 2018, an infant was observed resting in a crib and the crib was pushed against a cubby on one service side and the opposite side was less than 30" from the next unoccupied crib.

Plan of Correction: Director corrected during the inspection.

Standard #: 63.2-1720.1-B-2
Description: Based on record review, in six of seven staff records reviewed, the licensee did not ensure that staff submit to fingerprinting prior to employment. Evidence: During the inspection conducted on October 30, 2018, the licensee had the following staff persons receive orientation training and training related to the center prior to obtaining fingerprint results: Staff #1-hire date 10/22/18 and training was conducted on 10/3, 10/4, 10/8 and 10/9 Staff #2-hire date 8/31/18 and training was conducted on 8/27, 8/28, 8/29 and 8/30 Staff #3-hire date 5/14/18 and training was conducted on 4/30, 5/1, 5/2 and 5/3 Staff #4-hire date 10/15/18 and training was conducted on 9/28, 10/1, 10/2, 10/3 and 10/9 Staff #5-hire date 10/26/18 and training was conducted on 10/22, 10/23, 10/24 and 10/25 Staff #6-hire date 10/1/18 and training was conducted on 9/19, 9/20, 9/21, 9/24 and 9/26

Plan of Correction: In the future with new hires we will definitely make sure that we have their fingerprint background check before training.
