Comments:
The licensing inspector conducted an unannounced renewal inspection on Thursday, October 25, 2018 from 2:30pm to 5:00pm to determine the center?s compliance with licensing standards. The center?s director was present and assisted the licensing inspector throughout this inspection. The census for today?s inspection consisted of 40 school-aged children and 5 staff. Upon the inspector?s arrival, two groups of children were preparing for snack and taking bathroom breaks. Other children arrived at the center while the inspector was on site. The children were later observed doing homework and during free-play activities. The staff were observed providing age appropriate supervision and having positive interactions with the children. The classrooms were equipped with age appropriate toys and supplies. During the inspection, the required postings were reviewed and determined to be in compliance. The following information was also reviewed by the licensing inspector: daily attendance, first aid kit and emergency supplies, menu, fire and shelter-in-place drills, and liability insurance policy. There were five children?s records and five staff records reviewed. Medication is administered on site and reviewed for compliance. There are multiple staff with current MAT certification and Daily Health Observation training on site. Violations from the previous inspection were checked for corrections and found to be in compliance. Description: Based on record review and interview on 10/25/2018, the center did not ensure the results of a fingerprint background check were completed for one applicant/licensee, hired prior to January 22, 2018, by September 30, 2018. EVIDENCE: 1) The record for Applicant #1 contained results of a fingerprint background check dated October 17, 2018. 2) During interview, Staff #5 confirmed the results of a fingerprint background check were received for Applicant #1 on October 17, 2018 which exceeds the September 30, 2018 deadline for agents/applicants hired prior to January 22, 2018.

Plan of Correction: Per the Director: In the future, the center will follow up with all background checks that are due prior to expiration date.
