Arrived 3:25pm Departed 5:20pm A Subsidy Health and Safety inspection was conducted on 10/30/2018 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection. Forty-seven children were in care today with five staff. The children were eating a snack, doing homework, and playing outdoors. ? New subsidy requirements became effective on October 17, 2018. The facility has not yet complied with the following requirement(s): 22VAC40-665-540; 22VAC40-665-770 ? The facility is to review the new requirements and work with their assigned inspector to ensure future compliance. Thank you for your cooperation. Standard #: VENDSUB-000-012-E Description: Based on the review of six staff files the facility failed to insure that all staff hold first aid and CPR certification. Evidence: 1. There is no documentation of first aid or CPR certification on file for staff #5. Plan of Correction: The staff member was enrolled in a class, but was not able to attend. Another class will be scheduled.
Standard #: VENDSUB-000-032-A Description: Based on a review of the emergency drill log and an interview with the director, the facility failed to conduct an annual emergency drill. Evidence: 1. An annual lock down drill has not been conducted. Plan of Correction: The site director will insure that a lock down is conducted within 30 days and annually thereafter.
