Comments:
An unannounced monitoring inspection was conducted from 8:30 am-11:00. There were a total of 45 children with 14 staff members present. Children were observed arriving to the program, eating breakfast, and engaged in group activities. A selection of staff and children records, medications and other documents were reviewed. The center has new ceiling lights and freshly painted. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center has not maintained a complete record for each child. Evidence: Child #1's record, date of admission 9/24/18 does not have documentation of physical exam and immunization record.

Plan of Correction: I have asked parents for the documents, they are new to the area and they are scheduling an appointment.

Standard #: 22VAC40-185-510-G
Description: Based on a review of medications, the center has not maintained a prescription medication in its original container with the prescription label attached. Evidence: Child #2 prescription medication was observed not in the original container with the prescription label.

Plan of Correction: We will ask the parent for the prescription box.

Standard #: 22VAC40-191-60-B
Description: Based on record review, staff were employed without a completed sworn statement. Evidence: Staff #1, date of hire 5/24/18 completed the Sworn statement on 8/17/18 Staff #2, date of hire 8/3/18 completed the Sworn statement on 8/17/18 Staff #3, date of hire 9/21/18 completed the Sworn statement on 9/27/18.

Plan of Correction: The former director is no longer employed. The current director will have new staff complete the form prior to employment.

Standard #: 63.2-1720.1-B-2
Description: Based on staff record review, staff did not obtain fingerprint results prior to first date of employment. Evidence: Five new staff records were reviewed, all five staff started employment before the center obtained fingerprint results. Staff #1, date of hire 5/24/18 fingerprint results dated 9/26/18 Staff #2, date of hire 8/3/18 fingerprint results dated 9/26/18 Staff #3, date of hire 9/21/18 fingerprint results dated 10/25/18 Staff #4, date of hire 6/18/18 fingerprint results dated 9/13/18 Staff #5 date of hire 9/20/18 fingerprint results dated 9/27/18.

Plan of Correction: The former director is no longer employed. The current director will obtain the fingerprints prior to employment.
