An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 12:20 PM and departed at 2:45 PM. A total of seventy-eight children were in care. Lunch service, transition to nap time and nap time were observed. A sample size of five children's records were reviewed. Injury records and medication administration were reviewed. A staff record review of thirteen staff records was conducted on 11/2/18. An exit meeting was conducted with the program director prior to closure of the inspection.
Standard #: 22VAC40-185-60-A Description: Based upon review of five staff records and staff interview, the facility has not ensured that each child's record includes all required information. Evidence: 1. The record for child 5 did not include the addresses for the two designated persons to be contacted in an emergency when a parent cannot be reached. 2. Staff 1 verified that the addresses were not in the record for child 5. 3. The record provided for child 4 did not include a parent signature on the parent agreement form. 4. Staff 1 verified that the parent agreement form was not signed. Plan of Correction: The facility responded with the following: The parents will complete the information needed this afternoon. All children's records will be checked to ensure that all information has been completed.
Standard #: 22VAC40-185-340-A Description: Based upon staff interview, the facility has not ensured that staff always ensure the protection of the children. Evidence: When asked how many children were in care (under supervision) in the Airflights classroom, staff 2 responded that there were seventeen children in care. There were actually sixteen children in care. Plan of Correction: The facility responded with the following: All staff will be reminded of the importance of knowing how many children are under their direct care at all times.
Standard #: 22VAC40-185-510-A Description: Based upon review of the medication authorization forms and review of the center's written medication policies, the facility has not ensured that the written medication policies are followed. Evidence: The written medication authorization form required in the medication policies for the facility for the administration the emergency medications of benadryl and albuterol to child 2 did not have completed the dosages of benadryl or albuterol to be administered to the child. Plan of Correction: The facility responded with the following: We will ensure that the required medication authorization form(s) are completed as required by our policies.
Standard #: 22VAC40-191-60-C-2 Description: Based upon review of thirteen staff records, the facility has not ensured that results of a child protective services (CPS) central registry search is obtained within 30 days of employ or continued employment is denied. Evidence: 1. The record provided for staff 1, employed on 8/27/18 and on duty during the inspection on 11/1/2018 did not contain the results of a CPS central registry search. 2. The record provided for staff 7, employed on 4/9/18 and on duty during the inspection on 11/1/2018 did not contain the results of a CPS central registry search. 3. The record provided for staff 8, employed on 3/19/18 and on duty during the inspection on 11/1/2018 did not contain the results of a CPS central registry search. 3. The record provided for staff 11, employed on 7/9/18 and on duty during the inspection on 11/1/2018 did not contain the results of a CPS central registry search. Plan of Correction: The facility responded with the following: A licensing inspector discovered that the CPS background investigations were not completed during a review of the staff records. There was a miscommunication regarding the emails received regarding the background investigations. All CPS background investigations form will be sent overnight to Child Protective Services on Monday, November 5, 2018. In the future, CPS background investigations will be sent within the first seven days of hire for staff.
