Comments:
An unannounced inspection was completed on 11/05/2018 from 10:00 A.M.-2:00 P.M. At the time of the inspection 15 children were in care with four staff and two administrators present. Five children records, five staff records, six injury reports, four over the counter medications and documentation emergency plan, evacuations drills, first aid kits, emergency supplies, playground, and posted required information were reviewed. Children were observed during free choice activities, painting, sensory bin filled with fall items, learning activities, story time, handwashing, lunchtime, diapering, and naptime. Standard #: 22VAC40-185-60-A
Description: Based on a review of five children's records, the center failed to obtain all required information. Evidence: 1. Child #5's file did not contain addresses for two emergency contacts. 2. Staff verified that the there were no addresses listed for the emergency contacts.

Plan of Correction: The administration will contact the parent this evening to get the addresses and add to the file.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the center failed to ensure that all playground equipment with moving parts or climbing apparatus have the required amount of 6 inches depth of resilient surfacing. Evidence: 1. At the bottom of two blue slides there was bare ground exposed where the mulch had washed away. 2. The fall zone area around the climbing structure had bare ground exposed where the mulch had washed away. 3. Staff verified that the ground was exposed and the mulch had washed away.

Plan of Correction: The administration will order more mulch to be added to the playground.

Standard #: 22VAC40-185-520-C
Description: Based on a review of diaper ointments and required documentation , the center failed to have a current parent authorization on file. Evidence: 1. Child 1M had a tube of Aquaphor Ointment with a parent authorization form that expired on 10/04/2018. 2. The ointment had been applied to the child on 10/09/2018. 3. Staff verified that the parent authorization was expired and that the ointment had been applied on 10/09/2018.

Plan of Correction: Staff have contacted the parents and were aware that the form had expired. Staff will follow-up with parent this evening to obtain a current authorization.
