The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on 10/17/2018 regarding care, protection, guidance and diapering of children in care. The inspection was initiated on 10/18/2018 and completed on 10/22/2018. On 10/18/2018, the inspector was on site from 2:42pm to 6:10pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. Please complete the ?plan of correction? and ?date to be corrected? for each violation listed on the violation notice and return it to me within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only. The preponderance of the evidence gathered during the investigation supports the allegation regarding the care, protection and guidance for children in care; therefore, the complaint is determined to be valid. The preponderance of the evidence gathered during the investigation does not support the allegation regarding diapering. See the violation notice for more details. Description: Based on interviews and documentation at the facility, the facility did not ensure the care, protection and guidance of children in care. Evidence: During the inspection, the licensing inspector interviewed staff and reviewed documentation, the following was determined: 1. The record of child #1 had documentation from a physician stating that the child was to be served two cans of a specified nutritious supplement each day while the child was in care. 2. It was revealed to the inspector that there were substitute teachers in the Toddler class on 10/11/2018 and 10/15/2018. Teacher A informed the inspector of the following: a. Teacher A had child #1 in their care on 10/11/2018 and was not informed of the nutritious supplement requirement for child #1. Teacher A did not give child #1 the nutritious supplement for lunch or snack on 10/11/2018. b. The parent of child #1 asked if the nutritious supplement was given and informed teacher A of child #1 having the supplement the morning of 10/11/2018. c. The parent of child #1 stated that the child is supposed to get the supplement twice a day and the second serving should be for snack or lunch. 3. A center representative informed the inspector that they gave the nutritious supplement to the child twice during the child's enrollment at the center which was between 10/9/2018 - 10/17/2019. The center representative also stated that they could not remember when they gave the nutritious supplement to child #1. 3. There was no meal documentation at the center for child #1 for 10/11/2018 and 10/18/2018.

Plan of Correction: Dietary supplements will be documented and recorded on medication authorization log.
Date, time and amounts given will be documented. Staff administering will sign medication form.
A daily note will be sent to caregiver documenting supplement consumption date and times.
Supplements will also be noted on food preference form.
